Un Service d’Urgence (SU) est considéré comme la porte d’entrée de l’hôpital, ouvert 24 heures sur 24, 7 jours sur 7. La hausse de fréquentation au fil des années explique l’encombrement des urgences, les délais d’attente importants et l’insatisfaction des patients. L’enjeu majeur des services d’urgence est donc d’améliorer ses performances et proposer une prise en charge optimale aux patients. Dans cette thèse, l’objectif principal est de concevoir un outil d’aide à la décision pour un SU afin d’optimiser la plnification des ressources humaines (médecins et infirmiers) et des lits qui sont considérés comme la cause principale de l’encomprement des SU. Un programme mathématique stochastique est ainsi développé et la simulation Monte Carlo a été utilisée pour le résoudre. Il s’agit du niveau de décision tactique. Le niveau opérationnel porte sur l’ordonnancement des patients au SU, où nous évaluons différentes configurations et règles d’ordonnancement dans l’organisation du SU, et le parcours du patient en se basant sur des indicateurs de performance. Par la suite, nous proposons une approche, basée sur le couplage entre le modèle d’optimisation stochastique et le modèle de simulation des flux, permettant d’intégrer ces deux niveaux décisionnels (tactique et opérationnel). Dans ce travail, nous prenons en considération les incertitudes liées à l’arrivée des patients et les temps de service. Le cas d’un Hôpital Universitaire Tunisien a été considéré comme cadre d’application.
