Selon le rapport annuel de la croix rouge et du croissant rouge pour l’année 2006, le nombre de catastrophes, d’origine naturelle et humaine, a augmenté ces dernières décennies dans des proportions importantes. Ces catastrophes engendrent souvent un nombre de victimes important nécessitant des interventions urgentes. Face à une telle situation, les moyens sanitaires classiques et de routines se trouvent souvent dépassés, et par conséquent inefficaces pour absorber cet afflux massif de victimes. Ainsi, la mise en œuvre d’un système de gestion hospitalier conditionné par une optimisation des différentes ressources médicales est indispensable pour sauver le maximum de vies humaines.

Dans ce contexte, nous proposons dans cette thèse, d’étudier le problème d’optimisation des ressources humaines et matérielles critiques à savoir, les chirurgiens et les salles opératoires en situation de crise. L’objectif est de traiter le maximum de victimes, autrement dit sauver le maximum de vies humaines. Notre étude comprend deux niveaux : (1) un niveau préparatoire qui consiste à dimensionner les ressources dans le cadre des exercices de simulation du plan blanc, et (2) un niveau opérationnel permettant d’optimiser l’ordonnancement des interventions dans les salles opératoires. Aussi, nous étudions l’impact de la mutualisation des ressources sur le nombre de victimes traitées. L’un des défis posés à la programmation opératoire en situation d’exception est l’aptitude à faire face aux perturbations. Dans ce cadre, nous abordons le problème réactif d’optimisation de l’ordonnancement des interventions dans les salles opératoires. Nous considérons diverses perturbations possibles telles : une durée opératoire qui dépasse la durée estimée, l’insertion d’une nouvelle victime dans le programme opératoire, et l’évolution du degré d’urgence d’une victime.

Cette thèse est menée avec la collaboration de plusieurs structures sanitaires publiques en France et en Tunisie. Les résultats expérimentaux mettent en exergue l’apport de ces approches pour l’aide à la décision

Dans un système hospitalier, le flux de patients peut être défini comme le mouvement des patients depuis leur prise en charge jusqu’à leur retour à domicile. A chaque niveau du processus de soins, plusieurs problématiques d’optimisation peuvent être traitées comme la planification des besoins, la prévision de la charge globale, la coordination des soins avec les différents acteurs. Le flux matière peut être défini comme le mouvement de biens (produits médicaux, para-médicaux, etc.) sur l’ensemble de la chaîne logistique hospitalière à partir des fournisseurs jusqu’aux unités fonctionnelles (services de soins, laboratoire, etc). L’optimisation de la chaîne logistique consiste à coordonner le flux de matière et le flux d’information tout en produisant et distribuant les produits au bon moment, au bon endroit et en bonne quantité et en respectant les exigences des clients, et ce à moindre coût. Les activités de soins et logistiques sont planifiées selon trois niveaux de décisions : stratégique, tactique et opérationnel. Dans un contexte incertain et dynamique, on distingue sur l’ensemble des flux plusieurs problématiques liées : (1) au nombre de patients à transporter, à accueillir et à soigner, (2) au temps de service qui peut évoluer en fonction de l’état du patient, (3) à l’évolution de degré d’urgence suite à la dégradation de l’état d’un patient (évolution d’urgence relative à urgence absolue), (4) à la quantité à réapprovisionner par période et par niveau (entrepôts, magasins, cross-docking, unité de soins). Dans ce cadre, je présente, dans cette Habilitation à Diriger des Recherches, mes travaux dans l’optimisation des flux de patients où je me focalise sur le service d’urgence et l’hospitalisation à domicile. Ainsi que l’optimisation des flux de matière dans le contexte des Groupements Hospitaliers de Territoires (GHT) et le transport des matières dangereuses hospitalières. La prise en compte des incertitudes et de l’aspect dynamique-temps réel dans les systèmes hospitaliers sont considérés. Pour résoudre les problématiques posées, des modèles et approches de résolution ont été développés : méthodes exactes, heuristiques, méta-heuristiques, méthodes hybrides (optimisation-simulation), approches stochastiques, dynamiques et à base de contraintes probabilistes et théorie des ensembles flous.

Résumé :

Depuis juillet 2016, les hôpitaux publics français ont obligation de rejoindre un groupement hospitalier de territoire (GHT), afin d’y développer coopérations, mutualisations et économies d’échelle.
Le projet consiste à fournir un outil d’aide à la décision pour la mise en place d’une logistique mutualisée au sein de ces GHT.
